We were told 1 tablespoon per ounce, but we were haqbing big constipation issues. We've since lowered it a bit. We put 2 tablespoons per 4 ounces, so I guess 1 tablespoon per ounce would answer your question! We tinkered and found the amount that helped his constant spitting up and reflux problems, and so far (about 2 weeks now) it's going just fine.

My pedi recommended putting 2 tablespoons per 2 ounces of breastmilk. She also told me to play around with it and if it were too thin then increase it or if it came out too thick to cut it back. She said not to use more than 1 tablespoon per ounce. Hope that helps.
We were told 1-2 tablespoons per ounce. I kind of "eyeball" it. And make sure it's thicker than normal but not so thick that I can't use rapid flow nipples.
