For breakfast he gets 2oz oatmeal mixed with 2oz fruit. But for lunch & dinner, he gets them straight - or mixed together (like sweet potato & applesauce together).
He never really liked the rice cereal, so he never gets that anymore...only oatmeal for breakfast!
Every day is different. Sometimes we mix, sometimes we don't - usually we don't because we don't feed cereals, sometimes we mix multiple fruits or veggies together, sometimes there are no purees and only whole/finger/steamed fruits and veggies, sometimes mixed with purees.
It depends on the food. Some, like squash or sweet potatoes, he'll happily eat as-is. Stuff like applesauce needs to be mixed both for texture (too runny) and tartness. Things like peaches, which he hates, need to be mixed to hide them. Carrots I've found need to be mixed with formula and oatmeal because they're too grainy and he chokes if he eats them without a thicker binding agent.

depends. I'm still starting out with my son, but with my DD I did things individually at first, then started mixing.
I did tend to mix something like sweet potato or pears with rice cereal because rice tended to make ym DD constipated. And according to my notes (which I'm just starting to go through as I work with my son), my DD would only eat broccoli at first if I mixed it with potato or fruits... which I find funny now because she LOVES broccoli so much that I have to hide it in the fridge else she'll get into it and eat the tops off WHOLE crowns.
If the food of choice is too runny, I add cereal to bulk it up. But if it's nice and thick as is, then I don't. Usually he gets cereal or oatmeal once or twice a day, but always mixed in his fruit/veggie. I refuse to pump any more for cereal!
I always mix fruit with oatmeal for breakfast. She's eating table food for dinner now, but I used to mix rice cereal in with her veggies at dinner. At lunchtime, I mix plain yogurt in with her fruit.

ETA: I use oatmeal instead of rice cereal though.
