GD- Can you tell me about the care you're getting from your OB?

I like my OB a lot (he delivered my son as well), but he does seem to drop the ball on a lot of things.  As long as I'm an advocate for my care then I seem to be taken care of it, but if not a lot of things don't get done right. 

So I'd like to know what your doctor is doing to get you through the rest of your pregnancy with GD--- extra u/s? NSTs? extra appointments?  anything extra?  This way I can go in knowing what kind of care I should be receiving and be able to speak up if I don't get it.  

Also, how is your doctor monitoring your glucose numbers that you're recording?  are you sending them in every week to a nurse? or just bringing them with you to your appointments to go over with your doctor? I'm asking because my OB doesn't have me doing anything.  I was told last week (26 weeks) that I have GD and I don't have another appointment until 30 weeks.  Nobody will be looking at my numbers until that time.

  • I've been sent to a nutritionist/dietician, and I track my blood sugar 4 times daily.  If I do not have an appointment that week then I am supposed to call them and report my numbers.  I haven't seen the dr. to find out if they do anything else special.

  • My doctor isn't doing anything differently as long as I am diet controlled, no u/s, no NSTs.  THey're just keeping a closer eye out for any issues.  I had 1 appt with the nutritionist (worthless) and no extra appts with my regular OB. 

    I fax in my numbers every week and they go over them with me every 2 wks at my regular appt.  I assume they would call me if there's an issue with my numbers, but they've been good so far so I don't know.  Also, if I have three bad numbers in one week (bad being over 105 fasting or over 150 after meals), then I'm supposed to call the doctor.

  • i saw a dietician after my diagnosis and got my monitor and was told about the diet.  then i met with the maternal fetal medicine specialist about a week later who looked at my numbers and then put me on medication.

    As for my OB, they just ask generally if my numbers are ok.  They also sent me for one ultrasound at 33 weeks and i will go for another next week.  i also have to go in for twice weekly NSTs which is just a royal pain.

    I have NST 2 times a week, started at 32 weeks. This happened after I couldn't control my fasting numbers with diet alone.

    Once I was diagnosed with GD I had an u/s & I have one every 4 weeks. My last one is 2/10. My regular OB doesn't do much, they are waiting for my last u/s so we can decide if I will be induced or have a c/s (my baby is large first in the 80% then 66% for weight. So I'm not going be going past the 39th week)

    The high risk doc gets my numbers via fax every week, they call if somethin is wrong.

  • I am seen by my OB every week, bring my numbers, have twice weekly NST, and I see my perinatologist/have growth scan every 4 weeks - I am in the same boat as Kie310 - my last growth scan is on Feb 12th, they will decide whether to do an amnio and induce, or c-section, or if I will be allowed to go to 38/39 weeks - but they told me no way would it be past 39.
  • I was diagnosed right after Thanksgiving.  I met with the Diabetes Center, dietician, and have a weekly appt with the OB.  They pushed it to every two weeks for a couple of appointments but my fasting #'s went up to between 95-100 and I am back to every week. So far, I am controlling pretty well through diet.  I am measuring 4cm ahead so I am going for a growth ultrasound on Friday, but he is also breech, so that could be why.  They have not really discussed anything else (NST's, or medication) yet.
  • I was diagnosed during week 28.  He sent me for an appointment with a dietitian and had me start testing my blood sugar 4x a day.  I just bring my numbers in to him on a weekly basis.  During the next 2 weeks I tried to control it with diet alone but was unsuccessful so I was put on oral medication (metformin).  Although the meds helped it wasn't quite enough so 2 weeks later, at about 32 weeks, my OB had me start lantus insulin injections at night.  That has been a big help with my sugar levels by the way!  Also at my 32 week appointment I had a growth ultrasound and the baby was measuring average in size.  A week later I had a regular OB appointment just to listen to heartbeat and get questions answered.  Yesterday was my 34 week appointment and I had another ultrasound to do a biophysical profile/BPP.  I can't remember everything they look for in a BPP but there are like 5 categories such as rate of breathing, general movement, flexing at the joints, amniotic fluid volume, and something else, can't remember.  Everything measured fine but due to the GD my OB wants to continue to do a BPP ultrasound every week from here on out just as a precaution.  I don't mind seeing the baby on the screen every week! :)  He doesn't plan to do any NST's, as he feels that the BPP's are more thorough and provide the mothers with much more reassurance than the NST's do.  Also, he doesn't like to let his GD mommas go the full 40 weeks, so he is planning to induce me during the 39th week.  Hope this helps!
  • I am type1, so I am insulin dependent. My OB is a high risk specialist, but has just been in contact with my endocrinologist. He pretty much leaves it up to her to make sure the diabetes end of things are under tight control. I email her my charts every two weeks and see her once or twice a month. I see my OB once a month, have monthly US scans, and after 34 weeks, I will have NSTs twice a week, hopefully delivering at 38 or 39 week range depending on how big she is.
