Insulin increase question!

Well, as some of you already know, I am a type-1 insulin dependent diabetic. My endo and I talk frequently and she has been going on and on and ON about how much more insulin I am going to need later as I get farther along. While I am okay with pumping myself up (literally- thank GOD I have the pump instead of injections anymore!), I was wondering how your insulin dosing ratios have changed?  For example, say you started out with a ratio of 1 unit:10g of carbohydrate, when did your dosing change and what were your new ratios like? I just want to have a basic idea of how they change and by how much- so far, my insulin needs have actually  decreased :), however, I have heard of mothers having to use 4x their original dosing ratio! Crazy! Any shared examples would be great! TIA!

Re: Insulin increase question!

  • I'm Type 1, insulin pump dependant also. And starting off my bolus carb ratio was 1;5 all day. Now i go from 1;4, to 1;3 back to 1;4 over the course of a day. and my basal's went from 1.4 all day to 12am--1.7, 7am--1.8, 3pm--1.5. And it's probably going to go up from there since my sugars still arent EXACTLY where they want it. . . . .THANK GOD for the pump.

  • Thank you! I was interested to see how this changed- I was originally 1:6 for bolus ratio, and my endo was very nervous that as my needs increased, I may be taking HUGE boluses, so she decreased it to 1:8, all day, and adjusted my basal rates as well so I was taking less insulin overnight and more in the evenings after dinner. I was on 1.0 all day, but she adjusted it to 1.0 from 12a, 0.7 from 4a, 1.2 from 7a, then 1.6 from 3p to midnight.  I can't imagine having better control with injections- my  pump has been a HUGE blassing in getting my BG levels under control- especially now! How far along were you when they adjusted your levels like that to address that higher need?
  • Thanks for asking this question as I was curious as well.  I'm Type 1 with the pump and find that it's helpful to hear real experiences to help me understand what may be coming in the next 9 months.
  • I'm not too sure when exactly they first initially increased everything. . if i had to guess i would say maybe 12/13weeks or so?  Also i forgot to mention that they lowered my target range to 80-100 and also lowered my sensitity factor from 20 to 16. Hope i helped some, feel free to ask me anything and dont be afraid to PM. . even if you just want to talk :)

Oh it's no problem! I had just been hearing these horror stories from my endo about women who started out with a ratio of 1:12, and finished with a ration of 1:3, and I couldnt help but think to myself, well hell... thats a LOT of insulin and my insurance already acts like I am pulling teeth to get a vial every two weeks lol. I kinda just wanted to know when to expect and increase and by how much... However, I still kinda feel like an oddball because my insulin needs are going down! I think maybe its all this snow- maybe my body is using sugar at a faster rate because it's working harder to trudge through this every day? Lol hard telling right now, but I am sure it will change as I get farther along...

  • I am type 1 diabetic too but dont use a pump so dont know if i am much help to you.  But I can tell you that my insulin doses were changed every few days but after around 35 weeks starting decreasing.  After delivery I became super sensitive to insulin and am now taking less than I was before getting pregnant.  Just keep monitoring your blood sugars.

Don't feel like an odd-ball. . . it's really, really common for your BG to run lower in the 1st and sometimes even the 2nd trimester. Also. . just like "regular" pregnant woman. . every diabetic pregnancy is different. . and i think sometimes the doctors forget that. . it's like "oh. . your diabetic? then your pregnancy is going to go EXACTLY like this" and tries to shove you into the "pregnant diabetic" mold. Which is totally silly because no pregnancy is EXACTLY alike . . and same goes for us diabetics :)
