For us we are getting:
matteress (we donated DD)
swing (I wanted one that has a cord to plug in so I gave the old one away)
bouncy seat (I lent ours to a friend and well it never came back)
new diaper bag
new bottles and stuff that goes with
and if it is a boy clothes but we pretty much kept all the furniture, playmats, exersaucers, pack and play, high chair and other items. We had people ask to borrow them but I was worried about not getting them back.
Re: 2nd time moms what are you buying this time around?
We are buying nearly everything again. DD's dad has almost all of the gear from her first few years so DH and I only have her old stroller (which no longer fits our lifestyle), carseat (expired), swing, sled, back carrier and clothes. We will keep and use the last 4 items which is a huge help but we still need all the major items.
Luckily we have many gift certificates left from our wedding in the summer and have already purchased a travel system, pack n play and high chair. Just a few big ticket items left to get.
We're getting new furniture b/c with my DS we got something nice that would work for many many years.
Other then that, we need:
Swing---husband tried "disassembling" the old one when we moved and it did Not go well
Clothing---even if it's a boy, the seasons will be way far off.
Diaper Bag---old one got a huge tear in it
New nipples for bottles---the ones we have work with the breast pump and we'll be using it again

Thats about it for us. Everything else is pretty much packed away from DS #1!

Carseat, nursery furniture (DD is still using all of hers), bedding, breast pump (sold my other one, I knew I'd want a new model if we had more), sit and stand stroller, probably a few bottles (changed to Dr Browns after DD was already taking 4+ oz, so we don't have any small ones), lots of clothes since this one is a boy!
All toys, exersaucer, playmat, bassinet & swing, are neutral, and we're borrowing a bouncer.
Really not much, IMO.
ETA: And a double video monitor! I need one now more than ever for DD, so I figure I'll just get a double.

If this is a girl, we'll need clothes.
We are getting a new stroller, diaper bag, clothes, more bottles/nipples, bibs and another Boppy.
I'd like to get a new swing that plugs into an outlet since this one devours batteries, but I don't know if I'll be able to talk DH into that one.
DS is getting new furniture, but I will probably get a new chair for the nursery and move/add some things around in there.

My DD will be 10 when this one comes so all her gear is long gone and we don't have anything from my stepkids because their mom had it all and I'm sure had gotten rid of it.

We are getting new bedding, new mattress, new clothes (different seasons), new stroller, bottles, travel swing. I think that's about it!
all nursery furniture ( crib,mattress,dressers,changing table, glider, bedding decor; DD#1's crib is a convertible so its her big girl bed now!)
* bibs, burp cloths, few new bottles, new bottle nipples, bottle warmer, new diaper bag
* some new clothes! We saved ALL of DD's and I weeded through them kept what was nice enough to use again but This baby will be born in spring adn its still cold here in the spring...DD#1 was born in the mid of summer and it was scorching hot! So I'm afraid the sizes won't correspond with the season. But we'll see!

We purposely got a mostly gender neutral items for DD, so we are pretty set in most aspects. The things we're going to get this go around (we're not finding out the sex) are a sling, cradle (ours was recalled), and boy clothes if it's a boy.
Other than that, we're focusing on DD's big girl room.

Just about everything. The only thing I have from DD (almost 5 yrs) is the crib, mattress, high chair, Medela pump and 2 Boppy pillows. We gave most of her other things away to other moms (old co-workers, etc) and they never happened to find their way back. My SIL has my infant car seat in her attic, which was manufactured in 2004 so it is now expired anyhow, and she also has my PNP, which she said she will give back to me but I may not take back depending on the condition of it (it's been through 2 of her kids and was kept up at the lake for quite a while). I threw all of my old bottles, bibs, towels etc away as well.
Also - this time around it's a boy, so we have to buy all new clothes, blankets, bedding, etc as I did absolutely nothing "gender neutral" the 1st time. Also need a dresser and changing table b/c we got rid of the old ones, as well as new swing, bouncer, sling, diaper bag (old one was totally trashed). Basically we are starting all over minus a crib and a few other things.

eta: swing (we gave ours to a friend) and some girlie clothes. most of the nb - 6 mo clothes i have for ds are fairly gender neutral, but it would be nice to have some girlie things too.
If it's another boy...then minimal gear- new bottles, binkies, a changing table pad & cover
If it's a girl- look out- everything pink under the sun. Clothes, blankets, hair ribbons, shoes, tights, bedding, dolls, maybe a pink princess phone?
But the only big ticket item that I didn't get with #1 was a glider or some sort of chair to feed & rock in. I attribute not having a comfortable place to feed to failing at breast feeding.
