I still need to get DS's fixed because they spelled my name wrong. I have to get the correction form notarized and I'm just too lazy to go to the bank.
I thought that we had to pay for it, so I ordered 5 copies and paid for them (maybe $30, but can't remember). A week after I got the copies, I got the "free" one in the mail.

same here. I have the request form all filled out and ready to go, I just keep forgetting to mail it or go in.

Yes, we had to pay for ours ($25, I think). It was required for DS' baptism since they consider a Baptism Certificate a form of ID.
I think it was $17 in Cali.
