My poor, poor DS has a terrible red, bleeding diaper rash! He always gets one when teething (his 4 incisors are cutting simultaneously, poor darlin'). This is by far the worst he's had. Desitin, Butt Paste and A&D have not helped. I know it;s not yeast so Lotrimin is useless.
Can I spray some Solarcaine numbing spray on his sore tushy for pain relief? I have a call into the pedi but they take forever to call back. Just thought I'd ask my local network of "experts"!

lots of nekkid time (works wonders) and just keep it as clean and dry as possible. There's some magical creams the pharms have behind the counter... name? but I find that using anything WITHOUT ZINC helps Ava's bum. vaseline to protect can help... but just lots of time without a diaper.
Nystatin - requires a script from your pediatrician but it cleared up the last rash DS had that was really bad! and it is super cheap I think my copay was $4
Also Aquaphor is helpful in healing up a diaper rash.

calmoseptine. it's otc but you have to ask the pharmacist for it behind the counter. it's what they use at hospitals for bedsores and other major skin breakdown. it's what they put on my sons butt when his skin was peeling off in sheets from being on IV vancomycin (and having constant diarrhea as a result).
it's zinc and menthol. it numbs pain (feels like heaven actually, cold and tingly but does not burn). protects from moisture, heals broken skin, and is anti-fungal & anti-bacterial.
$10 per tube. worth it's weight in gold.
eta: you can get it at any walgreens or cvs. and when you use it the first time you get the butt really clean and dry (good to give butt lots of open air time) then goop it on thick. Then every diaper change after that wipe gently and only remove the dirty stuff - you don't need to scrub off all the cream - that would hurt because it stays good. just smear on another layer and you're good.
