Our insurance apparently covers up to $15k lifetime maximum...so far we have only done testing which isn't considered treatment, so we haven't tested that theory yet.
As grateful as I am for that, it is really only going to cover the surgery DH has to have prior to our first cycle...so when we actually so our cycles, we'll be paying completely OOP for everything.

Our insurance covers 4 rounds of IVF. That said, we spent $6800 OOP on our first round when you add up the $3500 we spent to meet our deductible (months of IF testing and monitoring pre-IVF), another $1500 on meds that were not covered by our policy, $850 to freeze a sperm sample, another $850 to freeze our embies, and who knows what we'll be charged for ICSI since it was not covered in our IVF coverage. So, we're still OOP plenty on a single round.
NJ has mandated IF coverage if 50 or more people are insured by the same employer-
my insurance covers - all meds, appointments, tests/diagnostics, monitoring and IUIs unlimited; I get 4 IVF attempts (counted as retrievals) and full covereage for unlimited FETs if i pay oop for the freezing and storage.
I count myself BLESSED. we would not be trying if we didn't have insurance. we're saving for adoption.

When I went for my first appt with my current RE I didn't know any of this and the lady at the front desk told me after the consult, I almost started crying right there in front of everyone cause I was trying to figure out how we could afford all this.

i had no coverage, so this open enrollment, we switched to DH's insurance. We now have coverage for 4 lifetime IVF cycles and unlimited FETs. Comes with a pricetag though... and we're still paying $12,500 OOP for the year. Was worth it though, considering the alternative.

Mine covers 3 IVF's supposedly. However, the deductible is huge plus they only cover 80% of what they think it's worth so I certainly feel like it's OOP after paying about $15,000 so far. Also, since I had to re-order the meds for cancelled IVF's and stim extra long (thus needing more meds), they counted each re-order as a separate IVF so I am OOP on most of the drugs. Sometimes "being covered" is such a scam and misleading.

Co-pays on meds only if ordered through an approved specialty pharmacy (I paid ~$500 instead of $3500 for each IVF). This literally took effect 2 days before we ordered the meds for IVF #1, we didn't have any coverage at all before 1/1/09.
All testing, diagnostics and procedures were OOP 100%. My RE billed all ultrasounds as GYN, so my insurance covered them. I got very lucky with that since that reduced my cycle cost from $11k to ~$9k.
I also had to pay out my deductible ($500 + 10% after the $500) for my D&C when I miscarried. Our total between two IVF's and my D&C was $20k. We had another $1k or so from testing prior to cycling (SA's, saline sono, etc.).
Right now I am completely OPP, before July 1st my insurance covered 50% of IF treatment except for IVF. Testing was 100% covered as were meds, with a co-pay of course. If we had gotten to IUI's it would have been 50% covered by the insurance the other 50% up to us.

My insurance covers 90% of Dr. visits, blood tests and U/S. Everything else is covered by us. They cover about 10% of our meds and nothing beyond "testing".
100% OOP. Not even diagnostics -- we managed to get a few things covered in there, thank goodness. My insurance covered hysteroscopy but not HSG, for example. if it's billed as gyn. And it covers bloodwork if you go to LabCorp, so we had the big blood tests before IVF done there.

Very lucky to be completely covered for everything including IVF, FET and all meds. Actually, IVF coverage is something that's keeping us in Massachusetts. We wouldn't be able to do the treatments we're doing without it.

I have to say that's the best one I've seen for IF
I live in MA where insurance coverage for IF is mandatory. We have up to 6 fresh IVF's and unlimited FET's. I'm extremely grateful considering I'm currently undergoing IVF#4. There is no way we would have been able to afford 4 fresh cycles. I really am so grateful and know how lucky I am to have this coverage.
Illinois also has mandatory IF coverage. I think my insurance covers 4 IVF cycles, 5 if there is a live birth in between. I think IUI is unlimited. I'm pretty sure meds and tests are covered too. I just switched to this insurance so I'm a little unsure on all the details. I do know that we have a $1500 deductible and then they pay 80%, so we are oop for 20%. We are hoping to have already met our deductible because we had tricare (military insurance) and blue cross blue shield in October so tricare should have picked up our deductible since I had some major lab work and an mri done.
We have full Insurance coverage for IVF but very high deductables..1500 per person or 3000 for family. I reached that in 2 visits to the RE. No co-pays though once you reach the deductable which is great!
I live in a IF insurance mandated state (NJ)- However, my husband's employer (NJ) gets out of that coverage by obtaining insurance from Kansas.......so we are completely 100% OOP for everything. Which is why we haven't started IVF yet.

Our insurance covers 4 IVF or 20,000.00 lifetime which ever comes first. We have used about 6,000.00 this year on ultrasound appointments and IUI procedures. Out of pocket we pay for the meds only. Unfortunately after the end of this year this wonderful coverage goes away and we will be 80/20 split out of pocket and out of pocket cost for IUI's. We are taking a break due to the change in our coverage next year if this last cycle does not work. Boo!

For three IUI's, 2 IVF's, 1 FET, and 1 surrogate round we were completely OOP.
NOW dear, sweet DH has a job that covers 100% of IF treatment, up to $20,000, if we were to decide to cycle again.

Spoke briefly with RE's office the other day.
Apparently our insurance covers IVF, but will not cover all the testing prior to IVF.
I have to go in this week to get the specifics about our plan.?

Unexplained Infertility
