Usually something off the registry. But if I just can't find anything off their registry in my price range or whatever, I will usually get diapers and wipes and diaper ointment. Because you can't have too many of those (unless the kid is allergic to something.)
Diaper bag filled with little stuff: pacifiers, nipples, bottles, bottle lines, burp cloths, recieving blankets, grooming kit, outlet guards, onesies. That can get expensive, so any smaller combo of that stuff works.
If they have a registry, I'd stick to that. I would have been much happier if people had gone to my registry instead of the rouge gifts. LOL. I appreciated everything I got, don't get me wrong, but I ended up with a lot of nipples and NO BOTTLES.
If there is no registry, I go with diapers, rattles, bibs, and wipes. If there is a registry and everything is already bought, I go with diapers and small things for older babies like bowls with suction cup on the bottom.
Registry. I try to get something big as close to my budget as possible. If there are no items in that range, I get a bunch of small stuff - I try to do a theme. (bathing, feeding, or diapering...)
If there is no registry I usually stick to the basics - diapers, wipes, butt paste, triple cream, lotion. I figure it is all stuff they can stock up on because they go through it. I was surprised that I received very few of these types of items for my baby showers.

I only buy from the registry if its at Target (I don't live near a BRU). If they aren't registered at Target, then I usualy buy either a cute outfit or hooded towel/wash cloths.
