Take IBU about 30mins before your appt. It was a little painful, but not bad. I was a little dizzy and crampy for 2 days afterward.

I hated it for the first 5 months I had it, and now I couldn't be happier with it. I pretty much bled / spotted for the first 5 months.
You may have some cramping after insertion. My doc had me take 3 advil an hour before my appt to help prevent the cramps. I still had some menstrual like cramping, but nothing too bad.
ETA: I've had mine for 14 months now. My periods are irregular, but they always have been. They are very light, only need a pantyliner. I still get cramps sometimes, but nothing too bad. I think they are a little better than they were before Mirena.
I've had mine since December of '08. I got it put in about 6 weeks after my son was born. I spotted for about a week and haven't had a period since. A little cramping after it was put in, but that only lasted about a day. I LOVE it! I just check for my strings once a month and other than that I don't have to think about it.
Loved it. I had mine inserted 2 months PP and had it removed a few months ago to TTC # 2. I was told the insertion would be painful because I never dilated with DD (c/s) but it wasn't bad at all. Felt like having a pap w/ AF cramps at the same time. I cramped and bled for about two days and after that ... nothing. No spotting, no period, no side effects for about a year and a half.
Removal was a piece of cake, too. Didn't hurt a bit. My period returned 2 days later and has been very predictable ever since.
