I sent a patient to go brush her teeth... I told her the toothpaste is by the sink. She came up to me, mid-brushing, saying "this toothpaste tastes funny". Turns out, she pumped hand soap onto her toothbrush out of a pump labeled "Dial Hand Soap". Seriously?
