Had my first appt - a few concerns.

Had my first appt. today (I only saw the NP, not my OB) and I had a few concerns I wanted to run by you guys, if you don't mind:

1. They're insisting on basing my EDD on the "pregnancy wheel" even though I told them I ovulated on CD20. So, my EDD from them is July 2, when I know really it's July 7. Not a big deal in the grand scheme, but my next appt is my NT scan, which she said has to be done within a specific time frame (I think btw 11w and 12w). They're basing my appt for that off of their EDD (July 2). Is this going to potentially screw up my NT scan?

2. Regarding the NT scan itself, the NP was strongly discouraging me from getting it. She said (and I quote) "it's designed for people who would abort a down syndrome baby." DH & I would not abort, but I do have a cousin with Down's and I'd like to be able to mentally prepare and/or find support groups in advance if our baby is Down's. Given her attitude, though, she made me feel like a total tool for wanting to get the NT scan. Anyone else get this vibe?

Sorry to ramble, but I was just irked by these two things. Not a big deal, because it was just the NP, not my OB (who I LOVE).

  • I think the window is larger than just the one week because I went today at 11 and a half weeks but baby measured 12wks4days and they had no problem doing my scan.

    As for her comments, no one made me feel like it was a test specifically for wanting to abort a Down's syndrome baby. Don't let her get you down, this was clearly her voicing her sole opinion and it was out of place. I also had one because I wanted to be mentally prepared, so don't let her irk you :)

  • I understand your concern. My OB based my EDD on LMP even though I knew I ovulated late. I had a 6wk ultrasound (because I'd been cramping) and when they were able to measure the baby, they corrected my EDD based on baby's size (which was even a little later than the date I expected). Of course, this was mostly arbitrary since we weren't scheduling any time-specific testing. If you are very concerned, I would call the OB and first ask, "how important is it that this test is done within the specific time frame". If they indicate it's critical, then request an early u/s to date the pregnancy more closely.

    If the difference between your best-guess EDD and the doc's is only a few days, I wouldn't worry too much about timing. Sounds like it's about 6 days difference (right... you guess you O'd on CD20, but most docs assume 14?)... so if the test dates are critical, this could be a week difference between when you can schedule the test. Don't let the docs ignore your concern if this test is important to you.

    As for the NP who was very opinionated, you're right not to let it bother you too much. A lot of the NPs who work with my OB are a little rude, too. She may also have been instructed to discourage people from getting it because of the slight risks... anyway, if you know it's something you want, don't let it bother you.

  • The NT scan can be done between 11 weeks and 13w 6 days and it is not designed just for people who would abort a Downs baby. There are some things that it can detect from my understanding that they can treat early or at least prepare you for besides Downs. They will adjust your due date when they do your first ultrasound which I'm not sure if your having one before you would do your NT Scan, but that would help put you on track as well. Days are off sometimes from when you ovulate to when the sperm actually got to the egg so don't worry about them being off a few days :)

    She really said that to you...just for abortions? That is so unprofessional. What about just wanting to know & prepare? Sheesh. So sorry.?

  • I believe the test can be done from 11 weeks to 13w6d.  Not exactly sure of the start time but I am almost certain on the end time.  Schedule your appt. for the middle or end.  You have a say in what time your appt is.  Say I can only get off of work this day or that day, etc. 
  • Most docs will base your EDC on LMP unless it is more than a week off by u/s.  CD 20 is 6 days off so you are OK.  I ovulated on CD 20 as well but when I went for my first u/s I was measuring right with my LMP anyway (1 day behind actually).

    As far as the NT scan goes, I would be put off if someone said this to me.  I think just being able to mentally prepare, find a good specialist etc. would be worth knowing in advance.

  • The NT scan can be done between 11w3d and 13w6d, so you should be fine. My OB used the wheel and it put me a week ahead of where I was, and she thought I miscarried, even though I TOLD her I O'd on CD26. Sometimes they need to put down the wheel and listen to patients!!!

  • My OB is basing it on my LMP as of right now. She said once she meets with me thsi Wednesday she may need to order and U/S to determine a due date. I had a really long cycle

  • As far as the NT scan goes, my OB schedules it routinely.  An abnormal test can also indicate other chromosomal issues besides downs, and if it is abnormal, they offer further testing with chorionic villus sampling.  It's a personal decision as to whether you should get it, and the NP shouldn't make you feel bad for wanting to have it done.

  • I think what that NP said about that NT scan is for people who would abort is reprehensible. Not just because its patently untrue, but because what you or anyone else does with the results of that or any test is none of her damn business. If I were you, I'd say something to your OB about it.

  • 1)  so the window at the place in which i got the NT scan was 11w - 13 w. . . a lot of doctor's go by LMP . . so, i wouldn't worry about when you O'd and stuff . . go with the date your doctor set-up.

    2)  My doctor encourages her patients to get the NT scan . . for them it is an early detection tool and not one to encourage their patients to get an abortion . . 

  • The NP called me back with my NT scan appt. - It's Dec. 18th, I'll be 11w2d according to MY calculation (but 12w1d according to theirs). Hopefully it'll work out.

    Oh, and I told my DH what she said about the NT scan (it being only for abortive purposes) and he responded: "Well, she sort of has a point."

    Douchebag.
