First of all, make sure you're mixing the formula with water BEFORE you mix it into your breast milk. Or better yet, give breast milk first, THEN give some formula separately. That way if LO doesn't finish the bottle, you won't have to waste BM if you end up throwing it out.
Also, I don't think a welt on the forehead would be a sign of a food allergy. Usually babies who have a problem digesting dairy have symptoms like gas, reflux, acting unsatisfied after a feeding, arching back, etc. But definitely ask your pedi!
Our DD has a rash on her face and she started to get bad "stomachaches" (screaming in pain for hours each time she ate) and then started projectile vomiting every time she ate. Hope this is helpful.
My DD has a milk protein allergy. Her only symptom was bloody stool (that's right, diapers with blood in them - scary).
My friend's LO has a milk protein allergy. She had no blood in the stool, but was not keeping any food down and was often in great amount of pain (looked like gas pain to my friend).
I don't know anything about welts being a sign of a milk protein allergy.

DD would get a rash on her cheeks and around her mouth almost immediately, then we noticed it travel to her arms and belly. It almost looked like a heat rash.

OP, WHY are you mixing formula in w/your BM? This is not how to supplement if that's what you're trying to do.

My DS has a suspected milk allergy. We just started supplementing with Alimentum and he has mucus in his stools (no blood) and also white blood cells. Pedi thinks it is allergic colitis. That is his only symptom. We might have to switch to neocate if the Alimentum doesn't clear it up.
The nurse at my doctor's office told me I should mix some formula (water added first) with breast milk in the bottle to get my son used to the taste. I want to start to wean him, but he hates the taste.
