With a bakery? No. But I haven't lived here very long. We just went to the Pumpkin Patch on Sauvie Island in North North-East Portland. It was really neat. They have a website www.thepumpkinpatch.com But maybe someone else could help you out with one with a bakery.
Oregon Heritage Farm has pumpkins and apple everything. Not sure about the bakery piece. They are out Scholls Ferry a ways into the country. I've never stopped there but they always have a ton of cars there.

Portland Nursery is doing their apple tasting next weekend. I think they have cobbler and cider.
And there is a big apple farm/stand on Farmington rd... sorry, can't think of the name.
