Assuming they made it past a phone conversation then I think aside from them consenting to a background check - the interview your child gives them is very important. We are on nanny #2 because even though #1 was amazing - DS did not like her - they could never fall into a groove together.
We have had our nanny a month now, and love her. When I was interviewing, these were some of my biggest ones:
*Describe your infant experience.
*What would a typical day with my DS inlcude?
*How do you deal with frustration?
*Describe some difficult/challenging/frustrating situations you have had with previous families/children and how you delt with them.
Also, reference and background checks are a must. Driving records were a big thing to me. If they didn't have a good driving record, I refused to move forward with that particular applicant.

This is a good point. After we made our top choice, we had her come over for a "trial day" with DS to make sure they meshed before we officially hired her. We were around all day and just observed her interacting with him to make sure she was going to be a good fit. Gave us a chance to get to know her a little better before we officially hired her.
