Just make the 5 oz. and let your baby not drink the last 1/2 oz. But if you really want to only make 4 1/2 oz (I'm assuming it's formula you are feeding) it would be 2 scoops and a quarter of a scoop.
I bought a Dr. Browns formula pitcher and make 30 oz in the morning and then pour bottles as needed. It is great for the 1/2 oz situations or the times when DD is still a little hungry and could eat 1 more ounce. I think that they are around $10, but well worth it.?

We put 4 oz water and 2 scoops of powder, and it always makes 4.5 oz total.
This too. Once you add the formula to the water, it makes more than you really intend to. It was about half an ounce when I was making 4oz, but as I increase DS's bottles it's even more than that.
