Girls with MFI - I need your help (ticker in siggy)

A friend of mine is considering IVF because her H's sperm came back "10% of normal" after a vasectomy reversal he had. She knows I had to do IVF to get pregnant and wants to get together to ask questions about the whole process, etc.

At this time she believes IVF is the answer although I did not speak to her directly about everything, just from her email, so I am not sure if a doctor is leading her that way or if she is just jumping to that being her only option.

I did not have sperm issues, it was my old eggs causing the problem so I am kind of clueless when it comes to the sperm thing. Not sure what is a potential working with IUI or working with IVF sperm analysis result, but I do want to steer her in the right direction if she is jumping ahead to IVF too soon.

What made your doctor believe IUI or IVF was the better option, be it morphology or motility etc? Any other advice I should share iwth her, besides coming here =)

TIA

A lot of years and a million tears finally led me to you.
After 7 years trying to concieve, 3 failed IUIs and 2 failed IVFs, my third IVF was a success!
My Christmas baby turned into a turkey bird! Dillon Richard was born at 34 weeks, 5 days on November 28, 2009 after 10 weeks on bedrest for preeclampsia.

  • My DH has low morph (anywhere from 4-9%)..we were told that some sort of insemination would be our best choice...but then my DH had vericocele repair surgery so we are hoping that will change. Right now we will probably go with an IUI but we've been pretty upfront with our RE that we aren't interested in IVF so that may be why she hasn't really pushed it. She seems to think that an IUI would give us a good chance even with his low morph though, but especially if this surgery increases it.

  • I don't have too much experience with IUI's but from what I remember you need at least 5 - 10 million post wash sperm that are pretty good swimmers to do IUI's at some clinics.

    found this online

However, if the total motile sperm count at the time of insemination (after the processing) is less than 5 million, the chances for pregnancy are substantially lower.

If the total motile sperm count is below 1 to 5 million, success rates are very low.

IVF w/ICSI has higher success rates than IUI's

It's been a long journey. TTC since 9/06. multiple IUI's and IVF's and 4 m/c's. IVF#3 = BFP, twins, induced at 34w6d due to baby b passing away (no explanation). Delivered on 35w1d, Baby A - baby girl, and Baby B - baby boy, our little angel.

  • We are dealing with low morph and borderline motility. Right now, I am seeing a gyn who specializes in IF.  We are on our second round of clomid + IUI.  If this doesn't work, it is on to the RE for us.  My dr. said he thinks RE will probably do one round of IUI w/ injectibles or go straight to IVF.  I am 38, so they are pretty aggressive with us.

  • My husband initial S/A was poor.  He had 32.5 million count (best part of the whole analysis), 35% motility and 1% morph using strict method.  The doctor said they like to see anything about 20 million count, 50% motility and 4% morph.  He said we could do IUI, but we would have a better chance of success with IVF ICSI.  He said if he didn't think it was possible to get pregnant with IUI, he would come right out and say that.  So, we opted to give the IUI a try first.  (Keep in mind, we already have a son who is 2 & 1/2 --we conceived "naturally" but it took over a year).

Now, my H's number are better (see siggy).  Dr. Licciardi's blog covers a great deal of information regarding S/A.  Also, I would tell her to make sure they repeat the S/A before rushing to any decision.  My H is proof of how the numbers can change drastically in just four months!!!!

Good Luck helping your friend out!!

  • My first app with RE we talked about the 2 SA's in a row with  0% morph. He told us that abnormal shaped sperm could not get through CM and could not penatrate an egg - but - he said some patients that were waiting to start IVF got pregnant on their own, with 0% morph. I think its a longshot. If the morph is low (but not 0%) but the motility and count is good IUI seems like it could work, I got pregnant although I MC'd on a month where we only had 4 million count for IUI so it seems possible as long as some good ones get up there. Our SA's are always different, some months have been great, some awful, so she shouldnt go by just one. GNC has vitamins for men called Mega Men or something like that that has the same vitamins that are in the expensive fertility blends. Thanks again sooo much for my package and good luck to your friend!

  • Man that chart that was posted is so depressing. It just looks so snowballs chance in hell like.

We're dealing with MFI...going for IUI #3 in September. No more looking at charts for me.

    Sorry for the side line.

TTC since July 2007 a year on our own, 7 IUIs spread out over 2 years, all BFN IVF attempt #1 - September - antagonist protocol, called off CD8 due too poor response IVF attempt #2 - November - called off on CD11 due to low estrogen IVF attempt #3 - started stims Jan 25th...converted/cancelled Jan 31st...SWITCHED BACK TO IVF Feb 3rd! ER Feb 7th...lets DO this! ET Feb 12th, Beta #1 141, Beta #2 356, u/s #1 hb 141 bpm, u/s at 7 weeks 1 day no hb, missed m/c. So sad. IVF #4 scheduled for June 2011, last chance, this is it
  • the SA being 10% of normal doesn't really tell you what the problem is,  so it's hard to say, but that chart is really telling.

Our SA SUCKS. All three numbers (well, our morph is borderline). we were pretty much told we have a snowballs chance to conceive any other way other than IVF with ICSI.
