When to become concerned about milestones?

I'll preface this by saying sometimes, I think I know too much for my own good.  I'm an occupational therapist and I get super paranoid about development with dd. 

DD is 5 months old and hasn't rolled over yet.  Actually, she shows absolutely no interest in rolling over.  The only time she's even come close is when she was trying to suck her thumb.  That's about the only thing she's interested in.  She's met all of her milestones on the late end of "normal" but I don't see her rolling any time soon.  When should I really start to worry about her not reaching gross motor milestones?

  • I have no idea, but just wanted to say that I'm an OT too and I completely feel your pain.  I'm already watching developmental things with DS and he's only been home for a week!

  • Are you basing your milestones on her actual age or her adjusted age?  One of our little guys is rolling over, the other isn't, but is trying like hell.  They both get PT through Early Intervention.  They qualified due to gross motor delays.  She actually only comes 1x/month, but we're going to up that next time she comes.  I work for EI as a speech therapist so I know how hard it is to watch every little thing they do.  If you are concerned you could always have an evaluation done.  I'm trying to be calm about milestones just because they came so far already and really did have alot of obstacles to over come.

  • My 34 weeker just learned how to roll over a couple weeks ago, just shy of 8 months actual.  I, too, was worried.  But, he always manages to surprise me when things "click".  Now, he's sitting up unassisted (although very wobbly) and is trying to army crawl. 

    Don't worry - she'll get there!

  • My DS is 4.5 months adjusted and he just rolled...but prior, I didn't think he was close to rolling either, and now he can't stop rolling!!  I hope that she'll surprise you and start rolling soon!
  • Our pedi says to always give him 6 extra weeks to hit his milestones.  In general, DS, like your LO, hits his milestones on the late end of normal.  Rolling is an interesting milestone in that there is sooo much variety between kiddos.  DS starting rolling in his crib around 5 months but it wasn't until recently that he's started doing it while playing.  Even now he rolls reluctantly.  He'd much rather be sitting up.  I don't believe that rolling is a precursor to any other milestone, so I wouldn't worry too much about that one esp. since I believe they say rolling tummy to back is a 5 month milestone and back to tummy is a 6 month one.  You're LO still has time to meet those by her adjusted age.  I wouldn't worry unless her adjusted age has past 6 months and she isn't rolling, starting to sit on her own, or able to support her own weight when you have her stand.  Those are just my thoughts though!! I'm sure your pedi will be happy to talk to you about what's normal at your 6-month check-up.
