Right now I put my toddlelr in the cart and both twins in the Moby. When they out grow, I will likely put one in the hotsling, the other in the cart in the carseat and just do 2 small trips a week.

It depends on how much I plan on getting. I've done three basic things:
1. Use the DSNG and then put groceries in the stoller basket and/or carry a basket or some sort of bag (IKEA has big reusable bags).
2. Put them both in the cart in their infant seats--put one on the top and one in the basket. This works best in places with big carts, like Sam's and Costco.
3. Put one in the cart in their infant seat (on the top) and then carry one in a Bjorn (or other type of carrier).

I've done those and also the stroller pulling a cart deal.
Mommy to twin girls, Ashlyn & Fiona, born at 34 weeks due to vasa previa.
It sounds like it's really cumbersome and chaotic, but it has always been easiest for me to just put them in their stroller and then pull the cart along (unless i'm just picking up a couple things...then i use the stroller and the store handbasket). It was a lot more difficult when I used the DSNG because it was harder to steer that stroller with one hand. Now with my combi twin stroller, I can maneuver much more easily with one hand.
I would either put them in the stroller and use the space beneath to stuff my groceries (both the DSNG and duoglider have pretty decent space) or I would push them in the stroller and pull a cart next to me.

I've actually been wondering about this since I found out we are having twins. Weirdly enough a mom and her 7 month old twins got in line behind me at Wal-Mart today. She put them both in the cart in their infant seats--put one on the top and one in the basket. I told her she was inspiration for me, there is hope that i can one day go to the store with both of my babies!
If I'm at the grocery store that's near costco I use the double seat shopping carts (LOVE THEM!) and they both sit in the cart. When they were smaller and couldn't sit in the cart there were 2 other options - one in the snugride on the cart and one in a bjorn or both in the stroller and groceries in the basket underneath. I usually went for the stroller.

exactly what I do
I honestly don't unless I just need a couple things and then I throw it in the bottom of the stroller.
I use grocery shopping time as some "me" time and dh watches the babies.
I do one of two things right now.
Bring the double stroller and the huge "mommy clip" with a huge Ikea bag and put groceries in there + in the storage under the stroller...
or- put one baby in the cart in the infant seat and wear the other baby.
