If DD poops when we're out, I change her as usual. I put the dirty diaper into a wetbag, and put it back in the diaper bag (it's waterproof and smellproof).
Some types of diapers (fitteds, flats, prefolds) need a cover, and some (AIOs, pockets) already have one built in.
Most have a wetbag, a zippered bag lined with waterproof fabric, in the diaper bag to put used diapers in while out. When you get home, you just dump the contents into the diaper pail and wash the wetbag too.
