At what point do you need a language evaluation test?

I just got a phone call from my friend. Her ds is shy of 18m and the pedi suggested that he goes to speech theraphy since he does not say mom, dad but random things like bye, hi, no.

When do you have to start worry?

  • I'm far from an expert... but I think 18 months is too young to start worrying about something like this. Personally? I would start worrying about 2-2.5 maybe...

  • My DD is 19 months and is behind on her expressive language.  She doesn't say any clear words and only a handful of almost words (i.e bah for ball, bubba for bubbles).  She has great receptive language (understands what we say).  Our pedi believes the earlier the better if she does need help.  The first step is an evaluation and then the experts decode if she will need any speech therapy.

    Pedi suggested a speech eval at 15 month appt since DS has not said his first word.    EI was here last week, a couple days shy of his 16 month birthday and found that while he was at or above age level in most areas, including receptive language, his expressive language was at 11 months so he is eligible for services.   He will be starting speech therapy in a couple weeks.

  • My Mom is a SP and I asked her about this last week. ?She says as long as your child is saying something (even if it is just babbling) at 18 months, she would not be worried. ?If by 2 years your child is still not speaking very much or putting a couple words together in a sentence, then she would suggest getting it check out.

    She gets very annoyed when pedis tell their patients to wait until 3 years or older. ?I guess it happens all the time and puts those kids way behind where they would have been if they had gotten in at 2 years.

  • My pedi said if DS didn't have *any* words by 15 months, he would order a speech eval that would be completed by 18 mos.  If he scores low on the evaluation, he'll start early intervention speech therapy around 18 months.  We're at 14 months 2 weeks and still no words, so who knows.

    But - I also have a friend who is a speech therapist and she says that (1) boys generally begin to speak later than girls, (2) most kids who aren't speaking by 18 months catch up on their own by 2 years.  So she recommends starting therapy at 2 years, like pp said.

    Your friend's LO sounds within the normal range to me, since he is saying some words at 18 mos.  But if the pedi is worried, I would listen carefully to his/her recommendation.
