My oldest dd had severe reflux and she would often have throw up come out her nose. She couldn't breathe well either but I'd try to grab the aspirator and suck out what I could. She threw up while she slept but never out of her nose. I would elevate the top of the mattress she slept on so that gravity wasn't an issue and I laid her on her side to sleep, that way if she did throw up, it came out, she didn't choke on it.
My pedi said they can breath through their mouth but they forget to. The only way they generally remember to breath through their mouth is when they cry.
