I got this from my insurance:
"Please have your husband's doctor call with a
procedure code and diagnosis code to the
preauthorization department. If Varicocele
is the diagnosis, and is the underlying cause
of infertility, then according to the notes in the
benefit summary, surgery to correct should
be considered covered. An authorization would
be required. The provider can also request
what is called a predetermination from that
authorization department."
