I got this from my insurance:

"Please have your husband's doctor call with a

procedure code and diagnosis code to the

preauthorization department. If Varicocele

is the diagnosis, and is the underlying cause

of infertility, then according to the notes in the

benefit summary, surgery to correct should

be considered covered. An authorization would

be required.  The provider can also request

what is called a predetermination from that

authorization department."

is he that busy he can't call? are balls that complicated?

I think he gets more days off than I would know what to do with. He was out Thursday and Friday, and out yesterday. His nurse told me who would call today, but she couldn't tell me what time. I'm sure he will call when I'm away from my desk and cell.
