We clipped them before we even left the hospital. DS's nails grow very quickly and he was cutting his face. He hated the mittens, so we just cut his nails.
My only suggestion is to be VERY careful and to do it when he's asleep. DH's hands are a bit steadier than mine, so he does it most of the time.
At about 5 days old. His nails were so long that he scratched his face. I usually file them every couple of days or so. I do it when he is sleeping or very relaxed.
our ds wore mittens for a long time but we still cut his nails about every week or so. now they almost need to be done 2 times a week. his toe nails are worse- he has actually cut his nose with them! we let him watch tv and we can do it without any trouble, otherwise he is wiggling and moving like crazy.
I cut DS's nails about once a week and it is the easiest to do while he is nursing. He will lay on my Boppy so that I have both hands free. I then use one hand to hold his finger steady and my other hand to cut the nail. So far this has been working for us. My suggestion would be to start as soon as possible this way your LO will be adjusted to the process once they start moving around a little more.
