The only thing I'd suggest is if DC is going to daycare, label his/her bottles, clothes, etc. Have a bag ready with diapers, wipes, and any other necessities (diaper cream, lotion, whatever else DC will need) to take with you the first day.
Maybe call your boss and make sure they're ready for you to come back. The company I work for deletes all your system accounts while on maternity leave, so it took a whole week before I had email back up and running, because my boss didn't go through all the hoops to get things set up ahead of time (really no fault of hers, she thought she did it right, it was just way overly complicated).
I also wish I could visit the center for a day with DD to see what she does all day. We stopped in for about an hour but she was only 12 weeks so she slept the whole time. Now she's almost 11 months and I love the stories I hear from her teachers, but I wish I could just go there for a whole day and watch her play with her friends.
The first week back is very rough. It gets easier!
I got professional photos taken during my last week and bought a nice frame for one. It was nice to put it on my desk my first day back. Everyone that came to see me wanted to see pictures, so I had a recent, good one right there to show them! It did make me sad at times, but I loved having her picture right there.

Get as much organized and put together as you can now. I made a trip to daycare with DS the week before with all of his things that would stay there (a bunch of diapers, wipes, back up outfit) so I didn't have to bring that the first day.
Also, if you can, make some food now to freeze. You'll be tired those first few weeks back and having some home cooked meals already made will be wonderful.
But most importantly, relax and enjoy your final week. Get in a few naps. Go for walks with your LO. And maybe do something nice for yourself. I treated myself to a bunch of new makeup before I went back. Great investment since without it I look really exhausted
DS is starting daycare the week before I go back to work. I think it will be a little better for my emotional being if I deal with the separation anxiety before I go back to work.
I plan to do some shopping. I need new clothes . I've been off for a year and was pg for 9 months so I haven't bought new work clothes for a while.

Get all the 'stuff' organized...like a pp said get labels done, bags packed, make sure you have pics for your desk, etc.
And make sure to let work know you are coming back...I forgot this time around and they'd changed the code to the building while I was gone LOL.
