One thing the dr mentioned today was that (and I hadnt thought of it before) Noah doesnt spit up/gag as much when he has thickened breast milk. He wakes us up almost every morning by gagging on spit up (even though his head is elevated) because he nurses once over night, which obviously isnt thickened milk. So what do you think is the best way (if we have to) go from BF to EP?
I'm about to leave work so I wont be able to get back on, but I appreciate any ideas you have!

Just a thought, but why do you have to go to EP? Can you pump enough during the day to give him a bottle of EBM during the night?
I don't think I really understand your question...I just wanted to say that I think you are right that EP means exclusively pumping.
Are you asking if you should pump at night instead of BF and then provide the expressed milk to LO in a bottle? Or you could try to fit in an extra pump session during the day so that you don't have to pump at night but can still give LO a bottle.
