DH and I are going to the Bahamas without our son next week. I want to leave a consent for my parents to make medical decisions on our behalf while we are gone. Does this contain all the needed info? I know I need to have it notarized, but am I missing anything else?

To Whom It May Concern;

We voluntarily give our consent for Mr and Mrs X to make medical decisions on behalf of our child, XYZ, in our absence from Tuesday, May 5, 2009 through Sunday, May 10, 2009.

He will be OOT, so if something were to happen it would be at the ER or a emergency care place, not the regular pediatrician.
