I filled everything out at 35 weeks and turned it into the Dr. last Friday. They called and said everything is done and they will be mailing it back to my employer for me! I am soooo nervous to not get a check the first week... and then the checks only being a portion of my income. As it is- we seem to live check to check....
I am a teacher. I gave the district my paperwork in February. However, I knew when my doctor was planning to induce and I also knw that taking any extra days will not impact me. I will get the summer off and will return in the fall. Are they going to hold you to a specific date to begin your leave? I think I'd give them the paperwork soon.
I just sent mine in this week. My HR wants it at least 30 days in advance, so I was on the early side. But I knew everything on the form, so I figured I'd get it taken care of.

4 months ago. My work is very slow, and we needed to train a temp person, and work out payroll, est. My work doesn't even know they were lucky that I gave crazy long notice to them.
