IVF ladies w/ insurance coverage

Did your insurance cover ICSI?

I noticed in our approval letter, ICSI was not mentioned.  So I called today and asked about it, and my insurance told me I needed to fax over a copy of H's SA.  I told them it was over 2 years old, but she said that didn't matter.  She said they will review the SA and then make a decision for ICSI based on that.

Anyone else have a similar experience?  Were you approved or denied ICSI based on SA results?

  • We were told it was covered but i had to call the insurance company to find that out. I was told this before I even bought the insurance coverage so they didnt know the results of h's s/a

  • Based on my DH's SA we qualified for ICSI to be covered as well.  They had just changed the authorization criteria so he had to go see a urologist specializing in fertility prior to granting us coverage but they did cover it.  For our subsequent cycles, we didn't have to do anything special and they continued to authorize it.

  • ICSI was included in our approval.
  • ICSI is covered in our plan
  • Ours covered it without SA documentation, but I've heard that pretty common.
