Calling Insurance re IVF, help with ?'s

Sorry I had no clue how to title this post, lol, did the best I could.  Anyway....I'm getting ready to call our insurance co to get details regarding our coverage for IVF with ICSI.  Does anyone have a list of questions to ask?  We were able to look online to find out 90% is covered after our $500 deductable up to $10k but I want details on what exactly is covered in this bubble.  Here is the short list I have so far please help!

What is the pre-approval process?  How long does it take?

Are meds covered?  How much?

Are U/S covered?  How many? 

How does ICSI work into this? 

Our RE also offers a shared risk program, do I bring that up with insurance?  Thanks ladies!

  • I called the financial lady at my RE's office, she gave me the codes for ER, ET and the diagnostic codes and then I called my insurance co. and asked if those were covered.  You'll probably have to call the specialty pharmacy that your insurance company uses to verify meds coverage.
