Question regarding breech twin...

My baby A is breech and has been from day one, so I'm counting on a section. ?I'm fine with that.

I've been told over and over though that even if he flips (baby B is head down) and I choose to try a vaginal delivery, they'll deliver in an OR b/c of the risk that B could then flip and they'd need to do a section after a vaginal birth of baby A. ?

So this morning I'm watching The Baby Story on TLC, and they're delivering twins vaginally in an OR. ?Baby A comes out just fine, and then all of a sudden they're yanking baby B out by his feet. ?It just looked awful and mom was clearly in more pain. ?It just freaked me out because his whole body was out except his head in between contractions... ?Isn't that dangerous? ?Why wouldn't they do a section for him, or try to flip him? ?Sorry if this is naive, I just thought they wouldn't deliver a breech baby vaginally. ?I was wrong!!

  • There are some doctors that feel comfortable delivering breech babies vaginally rather than through a c-section.  I have heard that it can be dangerous, but I guess these doctors feel that it is better to deliver them that way then have to perform major surgery on someone.  I don't know....

  • My doctor also told me that she delivers twins with baby B breech as long as B is bigger than A.  Supposedly A paves the way for B.  In my situation B was transverse and bigger than A so once A was out the doctor manually turned B to make sure he came out head first.  Glad I had an epidural.

  • I have heard the same thing - as long as baby A is bigger than baby B, they can deliver the baby vaginally pulling him / her out by their feet.  Like the prior post said - apparently baby A paves the way for baby B.  Fortunately that wasn't the case for me - I can imagine that would be painful.

  • Ugh, the idea of a breech extraction scared the sh*t out of me.  My entire pregnancy Baby B was breech.  My peri preferred a breech extraction over c-section as long as Baby B was smaller.  I was always going to deliver in an OR just in case there were complications.  It ended up being that Baby B had a growth spurt in the last week and was too big to do an extraction so we did a c-section.
  • I saw that episode too and was watching it just horrified.  It looked absolutely awful and scared the sh*t outta me.
  • My Baby B was delivered by breech extraction. I had an epidural and honestly, I didn't feel a thing. I just asked DH if it looked horrible and painful (since I didn't watch it myself ;)) and he said, "No more than the rest of it" (e.g. pushing out Baby A). I researched what to do with breech Baby Bs and read articles about it in three different high-risk OB textbooks and they all concurred that it is safer than trying to turn the baby externally and similar to or safer than doing a C-section (in terms of fetal and maternal outcomes). My OB concurred. THe reason many doctors won't do a breech extraction is not because it's unsafe, but because they're unfamiliar with it and/or not trained in it. My OB actually said it was pretty easy. Baby B was out 7 minutes after Baby A and that's the time to cut the cord for Baby A, hand him to me, do an ultrasound, find out Baby B had flipped to breech yet again, reach in, and pull him out. (The OB was doing the ultrasound with his left hand and reaching in with his right; I was focused on Baby A so honestly I wasn't paying much attention, but DH watched and said it was pretty cool. :)) Honestly, for me the breech extraction was the easiest part of the whole delivery! ;) I'm just really glad I got an epidural after all.

  • And my OB let me deliver in the regular LDR room, which I was really thankful for. THere was an OR team, prepped and ready to go, waiting out in the hallway during my whole pushing stage. If a C-section had become necessary, they would've just wheeled me down the hall to the OR for that.
