follie size at trigger???

Today at my u/s I had one follie at 16 and another at 12.  I am cd 17 and using gonal f.  The nurse just called and said to trigger tonight w/ ovidrel!  I am excited but a little confused...I thought they'd like to see a follie around 18 or 20 before triggering.  I asked the nurse and she said no...and that the main follie would grow a little more before releasing. I go for IUI on Wednesday.  Does this seem right to all of you?  What size follies have you ladies had at trigger?  Thanks

  • I had a similar experience, mine were 14 and 15...what clinic do you go to? I know success can be had anywhere from 15-20mm (or higher) but I see most RE's trigger when they get to about 18. They also monitor your e2 levels so that could have something to do with it. Good luck.

  • I have triggered at 17.5, 20 and 24. ?My RE likes to see 18 but says they will continue to grow a bit after trigger. ?So long as my e2 is ok, I have triggered under 18.
  • I triggered last night because I was already surging on my own.  I had 2 follies, one was 21 or 22 and the other was 15.   I think they typically grow about 1-2 mm per day so maybe she's thinking it'll be at 20 when it ovulates?  I'm sure if your nurse told you to trigger, it's fine.  I'm a little surprised, but maybe you were already surging on your own?  Keep us updated!

  • Thanks.  I go to the Fertility Center of New England in Reading, MA.  I love my doctor so far, this just threw me off a bit.  Thanks for the info ladies :)
  • my doctor was ready to trigger last cycle when i had 1 follie that was 20x18.  this time around we triggered at 24 - but that's partly b/c i could not come in earlier for a follie check b/c i was out of town.

    Q: At what size are follicles considered mature?

    A: Many doctors monitor follicle development during IUI cycles. Most trigger when the dominant follicle is within a certain size range. While there is always some difference in doctor preference, the norms are unmedicated 20-24mm, clomiphene citrate 20-24mm, FSH-only meds 17 or 18mm minimum, and FSH+LH would be 16 or 17mm minimum. It is possible for slightly smaller follicles, 14-15mm, to contain a viable egg. Also, follicles continue to grow until they release, usually at a rate of about 1-2 mm per day. A woman may ovulate more than one follicle in a cycle, but the releases will occur within 24 hours. When hCG is not used, only follicles close in size are likely to release. The use of hCG induces ovulation in about 95 percent of women, and will get most mature follicles to rupture.
