Possibly stupid question re: Rex registration

They ask for "Mother's Maiden Name" on the form. Right below that they ask for "Father's Full Name." They mean my maiden name, right? Not my mother's name? I assume so because there is nowhere else to enter DH's info. But I'm being paranoid and want to make sure. Embarrassed

I remember withoutshoes asking that same question and everyone told her it was her maiden name, not her mother

Murph, is that on the Pre Registration form? If so, can you tell me where you're sending it once you fill it out? For some awesome reason, my OB gave me this form at my very first appointment and I have no clue what the instructions they gave me for it are now that 6 months have gone by (other than them telling me to send it in in my third tri).

I'm cranky lately...sorry. By the way, we're due 1 day apart---there's a chance you'll hear my screams!

Yes, that question is on the pre-registration form.

The pre-reg form is at the back of the booklet for the Rex Women's Center. You need to submit the form, along with a copy of your insurances card(s), via fax or mail to Rex. The specific info (address and fax number) are at the back of the booklet right next to the pre-reg form. Also, it says you have to have everything submitted 30 days prior to your due date.

You go to Atrium, right? I go there as well. They'll give you another booklet if you ask. I had to ask because I'd misplaced mine. They were really nice about it.

I can also e-mail you the fax and mailing info if you have the form but not the booklet for some reason. My e-mail is kea192 at yahoo dot com.

I didn't have to send copies of my insurance cards. I also got a confirmation call and postcard saying that had received my complete information and I was officially registered.?

Oops, answered that one wrong! haha! I did have to submit my insurance card.
