Definitely tooth related. My son does the same thing, and I'm always going to the pediatrician just to have him tell me he's teething and pulling at the ear/s is normal. Even if he gets a fever, it's still most likely tooth related. I give my son some baby Tylenol and a frozen Gogurt to suck on.
I just flashed back 20 years to babysitting a girl who pulled her ear when she was tired. I totally forgot that is one of those signs, like rubbing ones eyes. My kids never played with their ears but I think it's pretty cute!
Doesn't teething suck? I was SO lucky my son was ONE before he got his first tooth. My DD got 4 by 9 months and then no more for a long time. She was fussy but is such an easy kid it was like, by the time we figured out what was going on they were already in.

Though even now, J will say his ear hurts and pull on it, but then if I ask him 2 minutes later if his ear hurts, he'll so "Nope!".
But for your J, definitely tooth related. Orajel might help (as you probably know!).
