The infant insert is basically this padded blanket looking thing.. you could totally use a blanket. When your baby is not 4 months yet, he/she might be too little, so you need to pad the babe a bit so its a nice fit. Also, you need the support if you want to put the baby in sideways. Izzie was born in the winter, so she always had bulky winter clothes on anyway, so I stopped using the insert (or a blanket) after a couple months.
I have one and used it. However, when I would put her in it, a leg would turn blue/purple! I asked the doctor about it and he said it was normal but to watch out for it... after it happened twice, I put the ergo away for awhile (funny, I found the moby at this time). I only recently started using it again now that she's bigger and it's much, much better... we don't need the insert anymore...
I never used the insert - Cami was 7lbs 11oz at birth and I began placing her sideways or fetal position in the Ergo when she was approx 2 weeks old. ?The insert seemed like more of a hassle and made it more bulky. ?
