Please help me out! I have a one month old (adjusted to one week) with a raw bum. She's very nearly bleeding. We have been CDing during the day with Grandma El's and using Pampers Swaddlers sposies at night with a "Butt Spackle" recipe recommended by the NICU nurses (Desitin, corn starch, Mylanta, and A&D ointment)... I don't know what I'm doing wrong here!
During the day we're mostly using BumGenius Newborns (and love them so far, no leaks, seem to be working for us) and GMD prefolds and covers. We have, but haven't yet tried (they're in the drawer), California Baby Sensitive Skin Diaper Cream and a sample of CJ's Butter. We've been using sposies at night, just because we've been a bit overwhelmed with twins, trying to get used to the "routine" - but if you can swear to me that the sposies are a part of the problem we could probably manage to go full time CD.
Our girls are really tiny right now (the other baby doesn't even really fit into her BG Newborns yet, we've been using LilJoeys and GMD Preemie prefolds on her), however we are willing to try another dipe if needed.
The red bum was starting before we transitioned to daytime CDing, but just in case... All CDs were prepped per manufacturer's instructions. We use Method Free and Clear (as previously approved by the CDing mamas here) and no additional laundry detergents/softeners. We use Method F&C on their clothes too, with no allergic reactions.

Eta: How long are you going between diaper changes? Some babies are just a lot more sensitive to wetness than others. The suede cloth helps but there is still a little urine against the skin. Also, are you using anything as a barrier when they are in the cloth? Any signs of thrush? Does it look like it could be yeast related or just regular old dermatitis? Sorry for all the questions.
We struggled with a rash that kept coming and going (and also started prior to CDing, but didn't clear up after starting CDs full time either). I tried every CD safe diaper rash cream I could get my hands on with no luck. I finally went to the ped and he said yeast, even though it didn't look like your typical yeast rash. I treated him with nystatin and sposies for a week and disinfected all my dipes and accessories and that cleared things up. Hopefully it stays gone! Now I'm using Baby Bottom Better at every diaper change because it has calendula in it (a natural yeast fighter) and it's a decent barrier cream. Thirsties booty love also has the calendula.
You might want to give your ped a call and see what he says. Or give one of the calendula creams a shot. Prior to this, Grandma El's was my favorite barrier cream (LO's bottom looked AMAZING...right up until the rash returned, haha), but the petroleum in it feeds yeast, so I'm a little nervous to go back to it. If it is yeast, you'll have to treat all your dipes too!
