Trouble TTC

Well, looks like I have a DX now...

The dreaded (but expected) "Unexplained Infertility".  I know I don't *want* there to be more - but honestly, I kind of wish there was something else, so at least we'd know why and what would work!  What really surprised me was he said he wasn't sure why the original OB (I don't go to her anymore) told me I didn't ovulate, I did - it was just weaker than he'd like to see.  

The RE was an interesting character but gave us some good information.  As for the game plan, he kind of left it in our court.  He said we could do 2 more IUIs with Clomid, then move on to Fermera+IUI, or we could go directly to Fermera without doing more Clomid.  I'm not sure what to do - so any suggestions would be great.  I seem to respond to Clomid (two mature follicles this round) so I kind of want to try another IUI or 2 on Clomid before moving on - but are there downfalls to this? Also, he said with Fermera he only does cd3 monitoring - no cd12 monitoring unless he combines the Fermera with FSH...is that normal?

Anyway - we have a game plan and a few days (14ish) to think about our next move if need be...

  • Unexplained Infertility super sucks. When I was diagnosed I was immediately happy because all my test came out fine, and then my RE grounded me quickly by saying that made it harder because there was nothing she could fix. 

     No advice on medication. I've only done Clomid.

  • I'm unexplained as well. Everything came back great, DHs numbers were above normal, and at first I was happy... But now it's like well then what the fluck is wrong!?!?

    I have no advice on the meds as I've only don't clomid. I did ask a similar question and got some great advice so hopefully those ladies chime in! GL
  • I am so sorry. We are Unexplained as well, and it sucks.Sad

    I just did my first IUI last week, with 100 mg. of Clomid. I responded very well to the Clomid, so we will likely do 3 cycles with Clomid before looking at other options. I know nothing about Femara, so I can't comment on that, but my Dr. said that after three tries of one course of treatment, the odds of success go down, so at that point, we would  explore other options. I think that if your RE is reasonably happy with how you responded to the Clomid, it wouldn't be a bad idea to try another cycle with that before switching to injects, but for me that is more a matter of cost than anything.

    Sorry I am not much help, but just wanted to let you know that I understand the Unexplained IF and am here if you ever want to vent!

  • I only did Femara once, on the highest dose. I didn't respond at all. You should still have a CD12 ultrasound with Femara. How else will you know if you're responding? If I hadn't had an ultrasound, I would be a nervous wreck right now. I'm currently on CD50 of my cancelled Femara cycle.
  • Our diagnosis is unexplained IF and it sucks.  

    If your RE doesn't do mid cycle monitoring, how will he know if you are responding to the femera?  Also I assume that means you will time your IUI with OPKs.  The only IUI I have done so far was timed with OPKs and it made me a nervous wreck wondering if our timing was ok.

    November 2014: IVF w/ICSI #4 Agonist/Antagonist with EPP and Prednisone, Baby Aspirin, Lovenox, and IVIG for immune issues.  Converted to freeze all due to lining issues.  2 blasts frozen on day 6!

    January 2015: FET #2 Cancelled due to lining issues

    April 2015: FET #2.1

  • we're unexplained as well, sometimes I hate it, sometimes I'm just glad we're both ok. I did the full 6 cycles of Clomid, and will move onto femara next cycle. my re told me the egg quality is better with femara, but I'm glad we did the Clomid first since I also responded well, I hate shots, and it was fairly inexpensive.

  • Thanks guys! I'm finding myself more frustrated by the dx this morning than I was yesterday. I guess the only thing keeping me calm about it is that we're on IUI #1 and DH has always been convinced that would be what works.  So, while I know I shouldn't put all my eggs in one basket, so to say, I do think we have a good chance this month. 

    Also, thanks to those who said Fermera needed to be monitored too - I thought it did.  The RE I'm going to is big on saving costs - but if monitoring needs to be done, then we'd happily pay.  Geesh, I thought going to an RE would be easier than an OB - they'd just do what needed to be done!

    How is your lining on clomid? If your lining is a concern then I would switch to femera. My RE likes to see it above 7 during treatment cycles and clomid always made my lining thin.

    9 is great! I might stick with clomid because you responded so well. How were your side effects? I think femera is essentially the same thing but other women have said on here that there are less symptoms with it. I've never taken femera before.

  • Yeah, your feelings are understandable, it's a diagnosis, but then again you still don't know what's wrong. Good luck with a new plan moving forward!

  • Hey. I lurk here but just wanted to say we were diagnosed with Unexplained Infertility this month.  HSG, Saline Sono, SA, BW all normal.  I was happy that everything is okay but now like you, I am thinking about it and wondering what will work for us.  My doctor says I will do 3 IUI Clmoid Cycles and then move on to 3 IUIs with injectibles, not sure if thats the Fermera you are talking about.  I should be getting AF this coming weekend so I am nervous to start this whole IUI process. If you respond to Clomid then I would say stick with that until your RE thinks its time for Fermera.  My RE said 3 cycles of something and then move on to something different.  Good luck!

    DX: Unexplained
    6 Failed IUIs (Clomid and Gonal F).  
    IVF #1 began August 2013- BC, 4 days of Estrace, stimmed with Gonal F and Menopur, Ganirelix, 9R 5M 5F, Medrol and Doxycycline, 5 day transfer of 2 early blastocysts- good quality, Progesterone, Estrace, and baby aspirin, BETA on 9/20 BFN,   IVF #2 began September 2013, stimming with Gonal F (higher dose) and Menopur, Ganirelix, 18R 14M 9F with ICSI, 5 day transfer of 2 BBs one was starting to hatch, 2 frosties: 1  BB and 1 AC, BETA on 10/21, BFN BETA was a 5- chemical 
    Getting some more testing and trying to figure out what the issue is before FET in December, started acupuncture on 11/10, RLP and some other BW nothing major to report except slightly high Prolactin. Prolactin was slightly high when I started with my RE and I already had an MRI which was Neg.  Endo biopsy=negative.  WTF is wrong with us???
    12/18/13 FET of 1 BB and 1 AC.  Hoping for a miracle.  Beta 12/26 Low BETA-8 2nd BETA 5- chemical
    After 3 failed transfers IDK where to go from here.  Still no real answers as to why this isn't working.  Getting a second opinion in February and looking into immune testing.  
    2/14- NEW RE- Immune testing showed a partial dq alpha match with DH- On prednisone and did intralipids prior to transfer.  3/14 Lupron, gonal f, menopur, HGH.  ER 15 mature all 15 fertilized!  5dt of an early blast grade 1 and an expanded blast grade 2.  BETA on 4/21.  1 grade 2 embryo frozen
    BETA #1 59 BETA #2 148 BETA #3 283 BETA #4 2,783! US at 6w2d shows 1 bean measuring right on track! HR 121.  US at 8w3d measuring on track HR 177. Released form my RE.  EDD 12/28
