need help w/ 3y/o

I have a 3yr old that I watch and 3 times now even though he is told constantly to get up and go to the bathroom if needed; he has pooped in his pull up, gotten up and changed it and pulls 6 to7 other pullups to wipe his hands with. I am at a loss as of what to do, nothing seems to work.
