Switching to Heparin

When you switched from Lovenox to heparin, what was your dose?

I am supposed to switch tomorrow and am kind of freaked out. The pharmacist told me he was nervous about giving a pregnant woman heparin. Oye.

My MFM told me that some pharmacies were having trouble obtaining certain concentrations, so he wasn't sure exactly how much I would end up needing to inject, so I should be sure to read the prescriptions and labels.

That coupled with the pharmacists reaction has me all nervous

I am going from 40 of Lovenox once a day to 1 ml heparin twice a day 10,000 units concentration. I think I will try to call my doc tomorrow too, but can anyone offer a reasonableness check?

  • Ive been on heparin fom the beginning. My doctor likes lovenox better strictly because it lasts longer but there is no reason to be afraid. Im on 7500 units twice a day but will be switching to 10000 twice a day when I hit 24 weeks. Best thing about heparin is no burn!

  • Also I pull 7500 from the a 10000 vial. 10000 should not be hard to find I get mine through CVS. Pharmacists are ignorant when it comes to Heparin. Trust your doc. Mine says she hands this rx out like candy and cant comprehend why my pharmacist isn't more aware.

  • I can't remember how much my dose was with DD, but yours sounds about right. My pharmacist was "concerned" as well. Not as much about the dose, but about a pregnant woman being on heparin. I know this is the standard though, not sure why they aren't aware of it, it's not that uncommon.
