Over the course of about 3 or 4 weeks I slowly replaced one bottle a day with a sippy cub (straw type). She was never attached to the bottle, so it really wasn't hard. She drank a little less at first, but not enough to worry, and eventually she drank more.

Can you start giving her a tiny bit of milk from the sippy? Then gradually when it time for more whole milk to add more, this way she can get used to the flavor of milk. It's possible that the formula/bottle thing are comfort to her.
Honestly-DD weaned herself. I didn't care about her having a bottle until she was 2. We did stop the ones during the day and she only had the morning and night one.
About 2 weeks ago she would drink from the bottle a bit and then put it down and move on. At that point I knew she wasn't attached anymore. We had been doing sippy cups for juice and water since she was 13 months and then we just went to sippys for her morning and night drinks 2 weeks ago.
