Has anyone done this recently? Times past I always just cleaned it out but DD is old enough now to know when something is missing even if she doesn't play with it. With Christmas just around to corner I would like to clean out her toy box of all the toys are are too young for her. How do you handle this in your house?

My LO is only 2 so he realizes with his "favorites' but I have always rotated his toys from day 1 so i think he's pretty used to it.
I keep his toy box in our basement where we play and then i keep a canvas tote in our storage (out of sight) with the "baby" toys that we occasionally will pull out but it is rare....maybe that's an option if you have the room?
I would try "you're going to get lots of new toys for Christmas but some kids don't lots of toys. Can you pick out some toys that you are done with that we could give to those kids?"
If LO doesn't pick enough I would take what you want to get rid of and put them out of sight for a week. Any that aren't asked about are gone. If one is asked about a lot, maybe keep it. If she asks where one is just once but doesn't keep on about it, I'd get rid of it.
