So after my most recent m/c my RE decided to do a bunch of blood tests for recurrent miscarriages. I am somewhat familiar with MTHFR just from my work (I'm a Neonatal NP), but realized I dont know as much as I would like. My RE said that our next step is going to be high dose folic acid/b-vitamin supplements, Clomid, Trigger, and TI (DH wants to wait a little longer until we start IUI). Anyone have any advice or experiences? TIA!

Do you know which type of MTHFR you have? I'm homozygous (so 2 copies) of C677t. From everything I've read, that is the most severe of the MTHFR's. Also, do did you get your fasting homocysteine levels checked? I've read a lot that states if your fasting homocysteine levels are normal, even if you test positive for MTHFR, your risks are not as great, and you *may* not even need treatment, as opposed to if your levels are higher/abnormal.
There seem to be two main issues with MTHFR (specifically with 2 copies of the c677t) from what I can tell: 1) Increased risk for blood clots (possibly causing early losses and even late losses due to clots in the uterus/placenta) and 2) decreased ability to convert folic acid into the usable form of methyfolate (causing neural tube defects, losses usually occur with anencephaly).
The jury is kind of out on the whole blood clotting issue. The most recent statements I've read from ACOG (and someone else) don't think that lovenox is required during pregnancy unless you also test positive for another clotting disorder. A lot of physicians will still prescribe lovenox in this situation (or allow you to take it if you feel strongly about this) because it won't really cause any harm either. Most just recommend a daily baby aspirin and that's it. And if you are heterozygous (one copy of the c677t and one of the A gene), they definitely don't recommend Lovenox during pregnancy (from what I've read).
The jury is also out on whether to prescribe megadoses of folic acid/b-vitamins (like Folgard) or to prescribe something like Metanx (sp?) that already has the folic acid converted into the usable form of methylfolate. I read somewhere (don't remember the article) that we only are able to convert/process 20-30% of the folic acid that we consume. So taking the megadose like Folgard allows you to convert the recommended 800 mcg.

Thanks for all the info! She didnt say specifically which 2 copies (I plan on getting the print out when I go in for an appt). She told me that I wouldnt need any anticoagulant therapy at this point. I'm starting the Folgard tonight. I'm sure I'll have more questions, thanks so much!
ETA: I didnt get a homocysteine level drawn, I'll bring that up with my next phone call to them.
