Maybe now would be a good time to bring up plans for future visits, so she knows you want her stay as involved as possible and she will continue to see her grandson regularly. Would your mom be able to Skype? Email videos/photos?
Just a suggestion: Start the conversation by bringing up your feelings first, "I'm sad we have to make such a big change and I'm worried about how we'll all adjust" to get her engaged in the conversation and then slowly start addressing some of her issues, so she doesn't feel ambushed and want to walk away.
Hang in there! She'll come around.
