Since I posted a long question below, I thought I should also finally intro.
I have been lurking for a few months and have mostly posted only on the loss board. My wife and I started TTC with donor sperm last summer and I was very lucky to get pregnant quickly. That pregnancy ended up being twins--baby A (Alice) and baby B. Both babies seemed great until our 14 week ultrasound at which point we found out that baby B had no heartbeat and had a large cyst growing out of his/her bladder which was taking over the entire abdomen. The cyst apparently prevented normal organ formation and was possibly caused by a chromosomal problem.
We were really sad about losing B as we had begun to think of ourselves as parents of multiples (even though we'd been scared shitless when we first found out it was twins). But we had another healthy baby to focus on and the doctors assured us that there was no reason to think anything would go wrong with baby A. But then when I was 18.5 weeks I went into preterm labor, though I thought it was just gastrointestinal distress I was feeling in my back mostly. By the time I realized something was wrong it was 8+ hours later. When we went to labor and delivery triage I was 4+ cm dilated (was told one only needs to get to 5 for a baby of that size) and my water broke at that point so there was nothing they could do to stop the labor. I did have an infection, though it's not clear if that caused the labor or if the infection started as a result of the dilating cervix. We had our anatomy scan the day before my labor and delivery and my cervix was measured then and looked totally normal so we know it's not IC. Alice was born on Jan 5th a little after 10pm 2.5-3 hours after we arrived at the hospital.
We are now almost 6 months out from our losses and it has been a really rough time. We just passed our due date with Alice which was harder than I thought it would be as we were also on our first cycle TTCAL during that time. So we are now on our second cycle and freaking out a little bit about getting the timing of insemination exactly right (as you can see by my post from a few minutes ago).

I'm sorry I don't have any knowledge to add to your insemination timing question, but I'm sure somebody will wake up soon with good info for you!
