DOR ladies - Ever had low P4 7dpiui?

I'm doing my first (OOP) injects IUI cycle, and just got my P4 level back. It's 4.24. They'd like to see it over 15, and my RE doesn't believe in progesterone supplements. The nurse said he wants to see what my body will do on its own, and that this'll help him understand what to do with meds if we have to do another cycle. (Doesn't sound so positive, does it?)

Even though I'm DOR, I thought I had responded well to Follistim (two mature follies at IUI, both measuring over 18mm) and that my chances for pg looked a little better. But I am now wondering if this means I actually don't respond well to meds.

Thoughts? Anyone else get low P4 levels and know whether it means much?

Began TTC #1 in Sept. 2010. I was 33, DH was 36.. Dx with DOR and slight MFI (8/4/11).

Test results (after first RE visit 6/21/11): 7DPO b/w: Progesterone = 11.3; CD3 b/w: FSH = 10.1 mUnits/mL, E2= 52.0 pg/mL, AFC: 6; SIS Ultrasound: Uterus great, tubes clear! AMH: 0.3 

IUI#1 June 2012 (Follistim/HCG trigger): BFN 
IUI#2 July 2012 (Follistim/HCG trigger): BFN 
IUI#3 August 2012 (Follistim/HCG trigger): CP (Beta #1: 4.61, Beta #2: 1.0) 
IUI#4 October 2012 (Follistim, ovulated before trigger, missed IUI, converted to TI): BFN 
Jan. 2013 New RE
IVF #1 June 2013 MDL Protocol. Converted to IUI#4.1. (High E2 and 3+ mature follicles at first monitoring.) BFFN
IVF #1.2 August 2013 Stop Lupron Protocol with Human Growth Hormone added. 17R, 14M, 9F (with ICSI), 2 embryos (decent quality - grade 2) transferred on day 3, 2 blasts made it to freeze. Beta 8/26. BFFN :'(

FET #1 December 2013 Last try! Transferred 2 blasts -- graded 3AB and 4BB. Beta #1 (12/27/13) 530. Beta #2 (12/30/13) 1876. BFP! One bean. EDD 9/3/2014!

We welcomed the most beautiful baby girl into the world on September 11, 2014!

  • I have low P4 and a short luteal phase (7-8 days).  I am not a doctor obviously, and I understand that there is some debate about progesterone supplements.  That said, I personally wouldn't actively TTC without them, for fear of miscarriage.  My RE believes that my January miscarriage was caused by my low P4 (or, my P4 could have been low because it was an unhealthy pregnancy to begin with).  My LP is also short enough at times that implantation may not have a chance to occur without them. 

    I don't know whether low P4 is related to DOR (I have low ovarian reserve also), but my RE did say that both can be affected by age.

    When I got a positive, but low, beta for DS2 my P4 was only 6.4 so my RE told me to definitely continue with the suppositories. It was my first cycle with follisitim, I had an 18.5 mm and a 18mm. I didn't have the DOR diagnosis then because I refused to do another clomid cycle for the clomid challenge test.

    Couldn't you over rule him and insist upon the progesterone supps?

  • Thanks for your replies, ladies. I started spotting yesterday and AF arrived this morning. So sad and frustrated.

    When I call the RE today, I plan to ask the nurse what they think happened and to get further explanation about why RE doesn't believe in progesterone supplements. (I'm told I would get a prescription with a BFP, if I needed it.) My mind is racing with questions like "Does low P4 indicate I have low egg quality in addition to low reserves?" I'd certainly like to hear his take on this before I start another injects cycle.
