Thank you gift for midwife

I was such a horrible patient, I think. And my midwife and birth assistant were so wonderful. What would be a good thank you gift? The midwife wasn't my sole midwife throughout my pregnancy (my birth center has 3) but I did end up seeing her the most, and the birth assistant I didn't meet at all until halfway through labor so there isn't even really anything personal I can include. I just want to send a card and a small gift of some kind.
