A Bumpie (I'm sorry I don't remember who) was going leave LO with relatives while they were out of town a month or two ago and she was making a binder of information to include while she was away (phone numbers, medical info, etc.).

I just wanted to follow-up to see what all she ended up including in the binder.  We're leaving LO with my parents for a few days next month and I'm looking for guidance in what all to include.

    We left the country for 4 days last month.  I just gave my mom C's insurance info, his pedi's number, and a notarized note signed by DH and I that my parents had full power over medical decisions for the dates we were gone.  The notary helped us with the wording.  

    I didn't feel the need to include any other info about his schedule as my parents are C's "daycare" and have kept him for the night before.

    Howdy! It was me! I put her

    • daily schedule
    • my DH and my numbers
    • medical power of attorney
    • insurance cards
    • located a pedi near my mom's house and downloaded and completed all of their paper work
    • placed their address and phone numbers in there as well
    • nearest urgent care facility
    • types of foods she has tried, things to stay away from
    • my DL # just in case
    • any prescriptions she was on and the pharmacy number
    • favorite games, toys, soothing methods
    • number of oz of BM/Formula and the times
    • instructions on how to mix
    • coupons for formula/supplies she may need

    I also brought her sample food items so she knew what to get just in case she ran out.

    My mom said it was VERY VERY helpful. It was a one stop binder that actually ended up working because she got croup the day before I left and I was able to go straight to the Pedi we had selected.
