I stressed myself out trying to come up with ideas. Just leave it all up to the photographer. Mine is fabulous and always gives the best ideas and pictures come out great!!
As far as what to wear, try to coordinate, but not completely match. You may want all to wear denim bottoms, but your mom may like a skirt, maybe you and DD in jeans, or DD in a jean skirt. Same goes for the tops, keep it simple. You don't want one person to stand out more than the other (for example, if you and DD are in white shirts, and your mom is in a hot pink one).
Your photographer should have a location they prefer. Otherwise, local parks can be great, or a college campus (though they usually require some type of permission first).

Like PP said coordinate but don't go matchy matchy. For example our last family photo I wore a dusty rose colored sweater, my husband wore a dark heather brown tee, and LO wore a tan jacket over a onsie with dusty rose colored flowers. We were all in jeans.
Some of my favorite pictures are in front of old barns/buildings (you might need permission), in the middle of a field - right now there are flowers, corn is great in the fall, railroad tracks also make for neat pictures.

FYI - If they're active tracks, not only is that incredibly dangerous, but illegal. Just so you know. I have no problem with track pictures, so long as they're inactive tracks.

I take pictures as a hobby and just did Easter pictures at a local church. They have white pansies that are in bloom right now and were gorgeous as a backdrop(click bio in siggy for pictures, should be last blog). We also had Spring family pictures done at a local college near the horticulture building where there were nice trees and flowers. MH and I wore white shirts and my daughter had a colorful shirt on and we all wore jeans, so she stood out a bit(blog with pictures under March) When we get pictures made, or when I do pictures, I always go on Yahoo Images and type in photography. I also look at local photographers websites to get ideas.
