1 - Nope. With DD1 our pedi advised starting with apples, bananas, peaches, and pears before moving on to orange veggies. She is a fantastic eater and loves her veggies, so the "if you intro fruits first they'll never eat veggies" was far from true for us. With DD2 we're going more slowly since she's less interested, and just introducing whatever we think might get her to eat.
2 - I have a 3-cup food processor (KitchenAide brand, I think) that is perfect for doing smaller batches of baby food. I freeze it in ice cube trays wrapped in saran wrap, then transfer to ziploc freezer bags. I don't think the Baby Bullet or Brezza, etc., are necessary.
3 - wholesomebabyfood.com is a great resource for making your own baby food.

No, banana was one of her first foods. We've done mainly veggies since then but that's because she usually eats solid food at dinner and we normally only have veggies.
A steamer basket is the only special tool I bought.

I did a combo of veggies and fruits at the beginning. I offered them in this order...squash, sweet potato, banana, avocado, pear, peas, green beans, plums, peaches, carrots, parsnip and I just added blueberries and chicken.
I make my own food and purchased some new ice cube trays...my old ones were pretty worn from two other kids. I also got a baby bullet from MIL when LO was born and I swear by it. When I made baby food for my other two I used my Cuisinart and coffee grinder (to grind the barley, rice and oats).
wholesomebabyfood.com has month-by-month charts setting out which foods are good to give LOs at what age. They go 4-6 month, 6-8 month, 8-10 and 10-12, and break down by what fruits, veggies, grains, proteins, and dairy are okay at which stage. None of it is set in stone, and every baby is different, but it's a nice reference, anyway.
We started with avocado and sweet potato. They say that feeding sweet fruits first can give babies a bit of a sweet tooth and make them not want the veggies later, but that hasn't been the case for us. The only thing we've tried that LO doesn't like is banana.
We didn't buy anything new. The only thing I'd recommend if you don't already have it is a steamer basket (we just use a wire mesh strainer, nothing fancy) and ice cube trays (we had 7 of them already!). And if you don't have a blender or food processor of some sort, you probably want one of those, too. I don't like the baby bullet, only because it's small and the little containers seem like they take up a lot of space compared to just using ice cube trays. Plus with ice cube trays, you can just knock the cubes out into a freezer bag once they're frozen. We have an old magic bullet, but I use my regular food processor for making baby food.

Did you go through all the veggies before offering any fruits? Nope
If you make your own food, did you get anything special for it(ice cube trays, baby bullet, etc.)? We have the Beaba and use silicone ice cube trays - Beaba is not necessary, but nice. Silicone ice cube trays are a must - makes it easy to pop frozen food out and store in ziplocs
